傳統分次 (Conventional fractionation) 與另類分次 (Altered fractionation)
為了提高放射治療對腫瘤的控制率,前人也實驗了各式不同的分次。其中包括了一天治療一次以上的高分次治療 (Hyperfractionation);一個禮拜治療不只五次、整體療程縮短的加速放射治療 (Accelerated fractionation);每次劑量超過 2 Gy 的低分次治療 (Hypofractionation) 等等。這些不同的分次方法,與傳統分次不同,故統稱為另類分次 (Altered fractionation)。
另類分次相當的多樣,有時甚至會將各種招式合併使用,例如很有名的 CHART (Continuous, hyperfractionated, accelerated radiotherapy),就是以一天三次、六日不休息、連續十二天的方式做完整個療程。
副作用相同,控制率提高
故在設計「另類分次 vs. 傳統分次」的臨床試驗時,另類分次的劑量往往會有所調整,目標就是在於將晚期副作用控制在與傳統分次相同。
統合分析 (Meta-analysis)
在這篇 2006 年的文章中,研究者發現,另類分次相較於傳統分次,提高整體存活的效果達統計顯著。但在當年分析之時,Median follow-up 只有 6 年;經過了十年之後,研究者希望更新資料,並加入更多臨床試驗做分析,故籌畫了這次 MARCH Meta-analysis 的更新。
分析方法
- 高分次治療 (Hyperfractionated):一天治療兩次,總療程時間不變
- 中等加速放射治療 (Moderate Accelerated):總劑量不變,但總療程時間縮短約一週
- 極度加速放射治療 (Very Accelerated):總療程時間縮短超過 50%,總劑量降低
若舉各組中最具代表性的試驗:
- Hyperfractionated:EORTC 22791
- Moderate Accelerated:DAHANCA 6&7
- Very Accelerated:CHART
當然,有一些另類分次方法比較難以歸類 (主要是運用 Hypofractionation 在早期喉癌的研究),研究委員開會後取得共識再歸入某一組。若該臨床試驗有好幾個 arm 比較,則採用傳統分次的控制組會被複製,形成多個一對一的比較。
研究結果
全部的另類分次一起與傳統分次比較時,另類分次可增加整體存活,HR=0.94 (95% CI 0.90-0.98)。五年整體存活的絕對差異 (absolute difference) 是 3.1%。
除了整體存活外,另類分次對於 PFS、Cancer mortality、Local failure、Regional failure 也都有統計顯著的正面影響,對於 Local control 尤其貢獻顯著,HR=0.79 (95% CI 0.81-0.98)。
另類分次在 Non-cancer mortality 與 Distant failure 這兩點上,與傳統分次無統計上顯著的差異,與預期相符。
高分次治療 (Hyperfractionation) 優於其他另類分次
相較之下,另外兩組對於整體存活的貢獻就無法達統計顯著;但中等加速放射治療 (Moderate Accelerated) 對於 Cancer mortality、Local failure 則是有統計顯著貢獻的。
另類分次 vs. 同步化學放射治療 (Concurrent Chemoradiotherapy, CCRT)
比較的結果,另類分次在整體存活上,顯著比 CCRT 來得差 (HR=1.22, 95% CI 1.05-1.42)。
貢獻最多病人的 GORTEC-9902,其另類分次方法為上述三組中表現最差的 Very Accelerated Radiotherapy。
在 CCRT 為主流的時代,另類分次還重要嗎?
研究者提出了一點,值得大家思考:在本研究中,Hyperfractionation 增加的五年整體存活為 8.1%;然而在化學治療的統合分析 (MACH-NC Meta-analysis) 中,CCRT 增加的五年整體存活只有 6.5%。單純以數字比較而言,Hyperfractionation 的效果甚至可能比 CCRT 好!
總結目前的證據,CCRT 優於單獨放射治療(傳統分次),另類分次亦優於傳統分次。在所有另類分次中,Hyperfractionation 表現最佳。Hyperfractionation 與 CCRT 之間孰優孰劣,仍有待未來更多研究。
不過說實在的,對於各種分次的效果,還有誰比放射腫瘤醫師更應該認真了解?
